Go to https://app.gabrielsoft.com/Account/Login on your phone where you will see the following screen. 

At the bottom of the screen, there should a bar where you can share this page.  Click on the share button where you will have a number of options.  You will want to choose Add To Home Screen.  This will paste a widget on your home screen.
